The transition of young people with complex chronic conditions from child health services to adult health services will be investigated in a new study.
The research, conducted by the National Confidential Enquiry into Patient Outcome and Death (NCEPOD), will analyse the barriers and facilitators facing young people transitioning between the two services.
According to the NCEPOD, they are looking to examine young people and care providers from a range of health and social care settings, including primary care, community care, physical healthcare, and mental healthcare.
Participants have until May 2022 to fill in the survey about their current role, organisation and day-to-day practice.
