This band 6 Diabetes Specialist Nurse role will include the following:
To provide specialist nursing advice, in a format appropriate to the individual’s needs/abilities, to professionals, people with diabetes and carers in all aspects of diabetes care.
Provide advice using evidence based/patient centred principles to assess, plan, implement and evaluate routine and specialist interventions with the support of a management plan.
To be accountable for safe and appropriate advice around medications used commonly within the speciality of diabetes care.
To support the diabetes team in planning, development and evaluation of diabetes services across the locality.
To be a resource for specialist advice and information across the organisation.
