An opportunity has arisen for experienced nurse to join the multidisciplinary diabetes team in Portsmouth.
Working autonomously, but as part of a progressive and supportive multidisciplinary team, the post-holder will provide clinical care and leadership for diabetes services across the acute hospital Trust, linking with community services to support seamless patient care.
Areas of clinical service provision across the team are: Care of inpatients with diabetes, Type 1 diabetes services (including use of emerging technologies and education to support this), young persons transition from paediatric services, management of diabetes prior to and during pregnancy, renal and foot services.
