Diabetes Specialist Dietitian

This is an exciting new post that offers an integrated role within the Self-Management Team and the Intermediate Diabetes Specialist Service as the Diabetes Specialist Dietician. The post holder will be required to work across and support both services to ensure service needs and KPI’s are being met. For Self Management, the post holder will be responsible for delivering the self management courses either face to face or virtually throughout Islington and Haringey: Diabetes Self Management Programme (DSMP) and Advanced Development Programme (ADP) alongside lay tutors. There may be some requirements to support admin and co-ordinators with the setup of face to face courses. Additionally, the post holder will support the planning and scheduling of the programmes for each quarter alongside the Self Management co-ordinators. Working across both services, the post holder will be expected to promote both services to patients, teams and other services to increase referrals and access into Self Management and Diabetes Service. The post holder will be expected to use their initiative to identify gaps as well as cross over opportunities for both services to align and integrate the services closely. Both services are keen to develop and continuously improve the patient experience.
