Diabetes Nurse

An exiting opportunity has arisen to share your care of diabetes by overseeing our diabetes patient register and arranging reviews and care plans by delivering excellent tailored care to our patients.

We are looking for a diabetes nurse to be responsible for the delivery of diabetic nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The diabetes nurse will be responsible for the diabetic register, diabetic care plans and management, diabetic health prevention and monitoring, as well as supporting the management team in the reviewing of clinical policies, procedures and patient care initiatives.

We are ideally looking to employ a diabetes nurse with Primary care experience although this is not essential. The candidate would be experienced in diabetes care, able to workindependently, have an understanding of clinical risk management, care planning and strong communication and IT skills.
